Dayne is now battling an addiction to both cocaine and the prescription painkiller Percocet. Dayne's mother, Shannon, is terrified that Dayne's violent temper and drug use will eventually get him killed. Josh assigned Dayne one of the most gut-wrenching tasks yet: spend a full 24 hours in the hell that could easily become his future. When Dayne refuses to follow Josh's plan, he is forced to draw a line in the sand—if Dayne does not comply, Josh will pack up and leave town.

When Ashley felt like singing had become a job, she stopped. She began using drugs to fill the vacuum. She is currently addicted to heroin and methamphetamine. Ashley's descent into addiction has also brought her into the world of a notorious street gang, where she has been mixing with criminals, been the victim of vicious beatings and has overdosed repeatedly on heroin. Her mother, Maureen, struggled to raise Ashley as a single mom but is now at her wits' end. Josh enlists the help of special effects makeup artists to give Ashley a glimpse into her future if she continues to use meth and introduces her to a former addict in an effort to give her the wake up call she needs. Will Ashley get back on the right track and continue her music career?

Asmara's mother, Sallou, doesn't know how to parent a teenager and her North African father, Michael, can't seem to see eye-to-eye with his very American daughter. Josh gives the entire family a wake up call when he makes Asmara spend a winter night on the streets with a homeless person and meet others who have heartbreaking stories to tell.

Since being adopted when she was 2 years old, Chelsea has battled feelings of abandonment and depression and now self-medicates with heroin. She shoots up as many as a dozen times a day, literally taking her life in her hands. Her adoptive parents, Deborah and Daril, are completely ignorant when it comes to addiction and have failed to set any ground rules for Chelsea, allowing her to live in a "drug addict's haven." In order to get Deb and Daril to relate to what Chelsea is going through, Josh brings the family to meet someone with a long history of abuse and personal tragedy and then, in a potentially life-defining moment, Josh arranges for an emotional reunion between Chelsea and a very influential person from her past.
